Conducting Personalized Assessments
Welcome to the HOC community! Our Certified Social Worker, Heather Hansen, brings years of experience and is committed to supporting patients and families throughout their care journey. Heather conducts detailed assessments for both pediatric and adult patients to gain a comprehensive understanding of each family’s unique needs. Topics covered include family life, financial needs, education, mental health, and more. Together, you’ll identify areas where additional support may be beneficial.
Connecting Patients to Resources
Heather works closely with patients to connect them with a variety of resources and support systems. These include financial assistance programs, community resources, and opportunities such as camps, advocacy days, and educational workshops. She also helps plan HOC’s patient and family programs, offering opportunities to connect and engage with others in the community.
